Emergency Travel Document (Qayıdış Şəhadətnaməsi)
Emergency Travel Document is issued in replacement of expired, lost or stolen passports to enable the applicants to return to Azerbaijan where a new passport can be applied for.
To obtain Emergency Travel Document in replacement of expired passports, applicants should submit the following:
- Original and photocopy of the expired passport
- 2 recent passport size photographs
- Application form (can be downloaded here)
- The receipt of the Payment of the state fee (10 USD) to the following bank account of the Embassy:

To obtain Emergency Travel Document in replacement of lost or stolen passports, applicants should submit the following:
- Police reference letter
- Original and photocopy of the National Identification Card (şəxsiyyət vəsiqəsi)
- 2 recent passport size photographs
- Application form (can be downloaded here)
- The state fee is not charged
Emergency Travel Document is valid for one month only. It allows the holder to travel only to the Republic of Azerbaijan. In order to obtain a new passport, emergency travel document should be submitted to the Passport Registration Office of the Ministry of Internal Affairs of Azerbaijan within 20 days after arrival.
